Affiliate / Visitor / Volunteer Request Form

This form is to collect requests from faculty inviting a guest that is classified as an affiliate, visitor, or volunteer. 
Please allow at least two weeks' processing time for your request. Questions regarding this form should be directed to SNS Personnel or your respective Department Staff.

To help us determine the required paperwork for your invitee, you can select "Unsure" to receive a series of questions to narrow down which appointment type is required. 

If you know what type (Affiliate, Volunteer, Visitor) you are requesting, you can select it by name in the list below. 

For the purposes of this request, we determine "research collaborator" to be someone working with you to write a paper or collaborate on a research venture.
