To complete your request to host a volunteer, please have the following ready:

Volunteer Information Form
Waiver of Liability Form
Patent Form (required if volunteer will be accessing university research facility)
- Note: volunteer to skip the Oath section of the Patent Form when completing
UCM Photo Video Audio Release Form (required if photo, audio, or video used in media)
Waiver for Minors in Labs and Shops Form (required for minors)
Activities Involving Minors Checklist (required for minors)
- Use Categories of Summer Camps Guide to determine if you need to complete the Activities Involving Minors Checklist
